  • Your first visit may run up to 90 minutes. Your doctor will perform a thorough physical exam and make treatment recommendations. The doctors will often do treatment ont he same day as your initial visit, especially if you’re travelling far.

    If you are interested in any regenerative injections, please be sure to email any imaging of the area prior to the visit. Although this is not required, it is highly preferred, since our docs may refer you for imaging before treatment. Please be sure to wear/bring comfortable clothing and allot 90 minutes or so for your first appointment.

  • Please refer to our pricing page for information on fees. Kindly note all fees are an estimate, and will vary depending on duration and complexity.

  • Prolotherapy, PRP and stem cell injections are non-covered services. Please refer to the pricing page for fee information. Trigger point injections may be covered by your carrier. The doctor’s time (history taking, examination, rehab & exercise/home care instruction, etc.) can billed to your insurance.

    Note, if you have a high deductible, you have the option of paying out of pocket, for which we will happily extend to you a 20% time of service discount.

  • If you already have images, it is preferred you bring them in. You can also call/email letting our staff know where you had the imaging done so we can request copies prior to the appointment. Otherwise, the doctor will order the imaging if necessary. Also, it is highly advised that you bring in copies (or grant access to a portal) of any lab tests from the past few years. In addition, please either bring in a full list of your nutritional supplements and prescription medications (a photo of the front AND back of all the containers will suffice).

  • Most patients can expect to be back to their normal level of activity in 2-3 days, although athletes will be requested to scale back the intensity for a time, depending on the condition and it’s extensiveness.

  • Occasionally, one treatment is sufficient for mild cases, but most frequently treatments are done in a series (4-6 weeks apart). Typically the doctors will treat 3-4 times in succession, then wait a few months to assess progress. Treatments may then continue, depending on the findings.

  • The treatment is surprisingly well tolerated. Local anesthetic is used liberally in the solution and the doctor’s skills are exemplary, both of which greatly minimize discomfort. After the anesthetic wears off the treated area may be more sore than usual for a day or so.

  • Unfortunately, we do not have an electronic health records system or patient portal at this time. Dr. Forwell does, however, offer a Fullscript portal, where you can reorder supplements. Watch this space!

  • There is a large lot directly in front of our building & spaces are generally readily available. The rate is $1/hour and is paid until 4pm on weekdays. Remember your stall # then pay at the green kiosk in front of the 1800 building (no need to put the receipt on your dash) or via the PayByPhone app.
